ChartHop forms let you create, secure, and share data collection forms with members of your ChartHop organization. For example, you can create a form to associate with a performance review. Or you can create a form that helps you deliver an organization-wide employee survey. Creating a form is the first step in creating any type of review for your organization.
To view and complete forms, your employees must have access to ChartHop. For example, you can't send surveys and review forms to contractors if they don't have employee access to ChartHop.

Before you can create a form, you must define any new custom fields that you want to use in the form within ChartHop. The data you collect in a form is stored within these fields. Learn more.
